..AA Insurance.
Renewal date arrives (mid May) and the "automatic" renewal of my car insurance form plops on the door mat. But I had already arranged new car insurance, so I phone up the AA and tell them I don't want to renew. Also cancelled the direct debit for the installments.
Looking at the bank account today, AA Insurance have taken over £420 - apparently as I cancelled the direct debit they took the whole amount despite the insurance being cancelled!
Rather an irate phone call to them and it's being refunded - too bloody right it is mate!!! 
Pete I thought the AA were supposed to be on the side of the motorist. Silly me! I forgot, it is now a business!
